The Opportunity
Reporting to the Manager of 6 Hematology/Oncology, Nephrology, and PACT, the Registered Nurse (RN) Long Term Follow up Care Coordinator is a member of an interdisciplinary care team, providing holistic, comprehensive evidenced based care to meet the needs of childhood cancer survivors with a primary focus on preparing adolescents and young adults for a successful transition back to community-based primary health care. The RN provides leadership in nursing assessment, and the planning, implementation, and evaluation of patient/family care for the purpose of monitoring health and managing late effects, supporting healthy psycho-social functioning, providing health promotion education, assisting with insurance, school, and employment challenges, and participating in survivorship research. The RN demonstrates an in-depth understanding of the unique needs of this population as well as available resources. The RN is responsible for educating the survivor about their past cancer diagnosis and risk for late effects from their past treatment as they age and grow. In doing so, the RN ensures each survivor has access to and is familiar with information as it relates to the implications for their ongoing health needs. This information is provided using principles of empowerment and self-advocacy to increase the survivor’s knowledge and self-efficacy for long-term health maintenance. The RN builds professional relationships with other nurses, health team members, and outside agencies to support the work of a strong interdisciplinary team and the provision of quality care. Using nursing knowledge, critical thinking, and clinical judgment, the RN ensures the goals and needs of the patient/family are prioritized and individualized. The RN engages in independent, interdependent, and dependent functions to provide health care focusing on comprehensive assessment, education, and coordination of care. The RN acts as a resource for colleagues working with children and families during long-term follow-up care. In addition to these responsibilities, the RN coordinates and participates in the nurse-led Survivor Clinic along with the Nurse Practitioner. The RN maintains a linkage with the Oncology Adolescent/ Young Adult National Task Force, which reviews survivorship issues for adolescents and young adults at a national level.
The RN helps to build our reputation for excellence in the Maritime community and beyond. He/she contributes to a sustainable health care system through formal and informal partnerships. The RN pursues excellence in care, teaching, and research through a spirit of inquiry, discovery, and innovation, leveraging our reputation and influence to advocate for health. The RN practices according to the Canadian Nurses Association Code of Ethics for Registered Nurses, the College of Registered Nurses of Nova Scotia Standards of Nursing Practice, specialty standards as applicable, and the policies and procedures of the IWK. The RN will be responsible and accountable for establishing and maintaining a leadership role as the liaison person between the interdisciplinary Oncology Service, other Health Centre services, and community resources in the delivery of family-centred patient care consistent with the objectives and vision of the IWK Health Centre.
Hours of Work
Monday to Friday; 7.5-hour shifts.
Your Qualifications
• Bachelor's Degree in Nursing required.
• Licensed or eligible for licensure by the Nova Scotia College of Nursing (NSCN).
• 3 years of recent pediatric Hematology/ Oncology experience required.
• Previous experience in patient/ staff education required.
• Current successful completion of a Heart and Stroke Foundation of Canada Basic Life Support Level (BLS) Program within the last 12 months, or successful completion of a BLS Renewal Challenge Certificate, is required.
• Oncology certification or enrollment in certification process an asset.
• APON Chemotherapy Biotherapy Provider Status required.
• Previous experience as a Family Care Coordinator preferred.
• Previous experience as a Clinical Care Provider is an asset.
• Experience working with adolescents and young adults preferred.
• Demonstrated experience in organization and coordination of care for patients/ families, including experience with transition of care, discharge planning, and inter-agency coordination required.
• Demonstrated leadership, communication, and interpersonal skills, with an ability to interact not only with interdisciplinary team members but also with community partners in health required.
• Demonstrated knowledge of growth and development as it relates to adolescents and young adults, and the unique needs of this population is preferred.
• Previous experience with clinical program development is an asset.
• Demonstrated ability to function in and manage multiple priorities within complex systems is required.
• Flexibility required for occasional weekend coverage in order to meet workload demands.
• Competency in other languages an asset.
